Dr. Shazia Siddiqui is the founder and leader of Lafayette Pain Care. Board certified in Pain Medicine, Dr. Shazia is an expert in treating acute and chronic pain in adults and children. She combines heart-felt compassion for her patients with knowledge of the latest treatments, such as spinal cord stimulation implants and pain medication therapy, including opiates. Her training includes:
- Fellowship in Pain Management Baystate Medical Center Springfield, Massachusetts
- Residency in Anesthesiology Jackson Memorial Hospital, University of Miami Miami, Florida
Dr. Siddiqui has published numerous articles on pain management topics. She is a member of the American Society of Anesthesiology and the American Society of Interventional Pain Physicians
Dr. Martinez is a native New Yorker who started his career caring for people and providing community outreach to the residents of Brooklyn, New York. From there, he attended Ross University School of Medicine where he graduated with honors. He continued his training to become an anesthesiologist at the University of Toledo Medical Center (UTMC), where he served as chief resident and was able to travel internationally and provide care through the Midwest Medical Missions. While at UTMC he completed his training as an interventional Pain specialist where he learned from national experts for treatment of acute and chronic pain conditions through cutting edge surgical and non-surgical techniques. Dr. Martinez believes in caring for people with compassion and providing up to date treatment options through multimodal therapy. In addition to caring for people, Dr. Martinez loves playing chess, traveling, listening to good music, and spending time with family.
Dr. Shahbaz Qavi is a board-certified Anesthesiologist and Pain Management specialist, trained at the highly reputable Cleveland Clinic and Cornell. As part of Cornell fellowship, he also trained in Hospital of Special Surgery and Memorial Sloan Kettering, ranked number one in Orthopedic Surgery and Cancer respectively. He brings the latest advancements in medical and interventional management of chronic pain to the Kokomo area. He has clinical and research interest in back and neck pain, joint pain, headaches, cancer pain, CRPS and neuromodulation. Dr. Qavi is a highly published author in numerous journals and textbook chapters, and has presented nationally at pain conferences. Dr. Qavi has been serving the Kokomo area for many years and is loved by his patients for his compassionate care.
Haley Innocenti, PA-C
Haley graduated from Indiana University in Bloomington in 2012 with a degree in Journalism with pre-med focus. She then worked as an EMT for two years to gain patient care experience. Haley then started her physician assistant program in 2014 at Indiana University in Indianapolis and graduated in 2016. Haley likes reading and playing with her dog in her spare time.
Jie Xu, PA-C
Jie Xu is a board certified physician assistant. He earned his master's degree of physician assistant from Mary Baldwin University in Staunton, Virginia. Xu has over six years of research experience at Indiana University School of Medicine. Xu's primary interest is in neck and back pain, peripheral joint pain, and a multidisciplinary approach to pain management. Xu is excited to bring his commitment to quality care to patients. Xu is also a member of American Academy of Physician Assistants (AAPA).
Laura Peacock, NP
Laura is a Family Nurse Practitioner who specializes in pain management at Lafayette Pain Care. She completed her Bachelor of Science in Nursing at Purdue Calumet in Hammond, Indiana and later completed her Master of Science at South University in Savannah, Georgia. She holds a national board certification with the American Associates of Nurse Practitioners. She has 24 years of nursing experience in a variety of settings including long term care, emergency medicine, family practice, nursing educator, and pain management. She works directly with the person who suffers from chronic pain to develop interventions and strategies to decrease pain intensity and improve physical functional ability. She utilizes an interdisciplinary approach to offer assessment, treatment, communication, education, and follow up.
